How are the topic difficulty ratings decided?

Each of the 21 topics is rated 1–5 for how hard it is to earn full marks in (not how hard it is to understand), based on where students most often lose marks in Cambridge exams. 1 is banker marks; 5 is a mark-killer that rewards exact mark-scheme phrasing.
